We are seeking a Team Assistant - Home Infusion Medford that will process and manage the health information records and data for a designated clinical team within Home Services. This includes accurately gathering, entering, compiling, reviewing for completeness and accuracy and problem solving patient information in the appropriate Providence Home Services information system(s) and in the clinical record. Communicates in a professional manner with public and staff.
In this position you will have the following responsibilities:
Obtain patient information referrals within established guidelines. Ensures referral information is recorded accurately, timely, completely and is routed to the appropriate clinician
Organize and process paperwork in an effect and timely manner. This includes checking itineraries of the schedule board, reviewing documentation for specific information as assigned, organizing documentation for expedient review by the team's clinical supervisor
Problem-solves missing or incomplete information, escalates deficiencies to the appropriate clinician and takes appropriate follow up measures to ensure the deficiency is resolved in a timely manner that meets associated deadlines. This includes auditing clinical charges and information prior to weekly/monthly invoicing, review of records for CQI
Provides support to clinical team. This includes but is not limited to:
Contacting clinical staff of late documentation
Monitoring and tracking assigned record management functions such as recertifications, kardex, case management therapies, etc
Notifying the appropriate staff member or clinical supervisor, of trends in delinquent or missing information
Accurately keys initial care plans or sets up treatment plans into the designated computer system in an accurate and timely manner
Provide clinical staff with certification, team conference, or other case management reports and documentation as assigned
Maintain referral source database in the Home Services computer system as assigned. This includes validating medical licensure and obtaining UPIN information prior to registering the physician or nurser practitioner in the referral database
Communicate concerns and needs effectively with others in a manner that promotes positive teamwork
Share and communicate with others in the department in a manner that demonstrates ownership and accountability for the outcomes of assigned processes
Ensure continuity of job functions by communicating coverage needs, priority and contributing to team effort to meet routine and relief departmental coverage in a manner that promotes a positive team environment
Assist in developing and implementing operational procedures as directed by the Clinical Supervisor and Manager
Assist with other projects and duties as directed by the Clinical Supervisor or Manager
Create and organizes patient's medical record documentation in an efficient and timely manner. Reviewing documentation for specific information as assigned, organizing documentation for expedient review
Assist with faxing, distribution of all received faxes, and other support functions to ensure strong referral access for HS customers. Manages the electronic referral system
Attend and participates in Home Infusion staff meetings and education sessions on a regular basis. Attends other meetings and education classes requested by the Supervisor, Manager, or Director
Required qualifications for this position includes:
Educational coursework in medical terminology -OR- equivalent education/experience
Two (2) years of recent experience as a unit secretary, clinical technician, medical office assistant or health information management
One (1) year of recent experience with multiple computerized applications required, electronic health record or business school level training and experience
One (1) year of recent experience in a team setting
Knowledge of generally accepted health care record keeping practices
Preferred qualifications for this position includes:
One to Two (1-2) years of college coursework and/or formal business or medical training
Experience with Excel and Word
Experience in a Home Services agency, medical terminology and ICD 9 coding or equivalent knowledge
