Providence Health & Services RN - Hospice in Medford, Oregon
Providence is calling a Registered Nurse to its Hospice team at Providence Medford Medical Center in Medford, OR. This is a full-time, day shift position. Our RNs work Monday-Friday from 8a-5p, and take standby/call 1 time roughly every 5 weeks. Our RNs do not work weekends unless on standby/call.
We are seeking a Registered Nurse who will be accountable for direct and indirect quality care for patients and families of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Accepts professional development as an integral part of nursing practice. Coordinates patient care activities with other members of the healthcare team in a collegial relationship to promote patient comfort and peaceful death.
In this position you will have the following responsibilities:
Under the direction of physician orders and using the nursing assessment and process, makes an assessment of the patient and family's needs to develop the nursing care plan.
Initiates the interdisciplinary problem master. Confers with physicians and other hospice team members to develop the interdisciplinary Hospice plan of care.
Evaluates on an ongoing basis the patient and family's psychosocial, spiritual, and physical needs, including the need for respite care. Initiates necessary referrals in a timely manner.
Teaches, supervises, and counsels the patient and caregivers regarding patient care in accordance with the patient and family's wishes including cultural and religious practices, emotional barriers, desire and motivation to learn, physical and/or cognitive limitations, communication styles, and language barriers.
Maintains patient clinical, progress, and itinerary records to meet JCAHO and Federal regulations.
Directs the care given by the Home Health Aides, respite care givers and volunteers by providing verbal and written instruction.
Updates the interdisciplinary plan of care every 14 days or as patient condition requires.
Informs the physician, members of the hospice team and other parties as indicated of changes in the patient's condition and needs.
Manages a nursing caseload as assigned by supervisor that includes patients requiring a broad spectrum of care, who reside in a variety of environments, and in a wide geographical area, by practicing good organizational and time management skills unless assignment is one to assist RN case manager by visiting or admitting their patients.
Rotates through weekend and holiday as assigned by supervisor and according to PPMC contract. Can participate in 24-hour call.
Performs procedures and treatments according to accepted organizational and department policies and standards (complies with all H and CS applicable policies).
Participates in the Continuous Quality Improvement process and other quality management activities as defined by the organization.
Participates as preceptor in the orientation of new staff and/or students as assigned by supervisor.
Required qualifications for this position include:
Education to meet certification, license or registration requirement
1 year Medical surgical experience, recent acute care -or- long-term care experience
Experienced in EOL care.
Valid Oregon nursing license
Current CPR certification.
Valid Oregon driver's license.
Preferred qualifications for this position include:
- Recent Oncology or Home Hospice or Home Health experience
