Providence Health & Services Medical History Pharmacy Technician in Medford, Oregon

Providence is calling a Medical History Technician Pharmacy - Pharmacy (0.9 FTE, Rotating/Variable) to Providence Medford Medical Center in Medford, OR.

This full-time position will work 10-hour shifts; 7 shifts on, then 7 shifts off. The shifts are typically 1pm - 11:30pm.

Excellent candidates will prove that they are dependable, have a strong understanding of pharmacy, are able to work autonomously within our Emergency Department environment. This position is a key role in the success of our program.

We are seeking a Medication History Technician (MHtech) - Pharmacy that will conduct patient, family or caregiver interviews to obtain a list of medications a patient was taking prior to admission. If needed, the MHtech may utilize other data sources to further investigate and complete an accurate medication list. The medication list compiled by the MHtech will be entered into the hospital's computerized medication reconciliation program and verified by a registered pharmacist. On occasion, may perform pharmacy technician job duties as assigned

In this position you will have the following responsibilities:

  • Perform medication history assessments for new admissions to the hospital, providing complete information regarding each medication identified (drug, dose, route, frequency, indication for PRN, and last dose taken-date/time)

  • Verify patient allergy information and ensure complete documentation (substance, allergy category, reaction description)

  • Coordinate medication history assessment

  • Document the medication history and allergy information in the electronic medical record accurately and in a timely manner

  • Participate in performance improvement activities

  • Participate in the training of Pharmacy Technicians, Externs, and Pharmacy Students regarding home medication interviews and documentation

  • In addition to these specialized responsibilities, the Medication History Technician will also be able to function as a standard pharmacy technician

  • Assist the pharmacists in preparing and distributing drugs and maintaining drug inventory

  • Knowledge and application of hospital wide compliance policies as well as specific compliance issues for area of accountability

Qualifications:

Required qualifications for this position includes:

  • High School Diploma -OR- GED

  • Must successfully complete Medication History Technician Competency Program

  • Current Oregon Board of Pharmacy Technician license

Preferred qualifications for this position includes:

  • Graduate of a Pharmacy Technician program

  • Experience with current pharmacy information systems (i.e. EPIC, Pyxis applications).

  • Two (2) years experience as a certified Pharmacy Technician in retail or inpatient
