Providence Health & Services LPN - Hospice in Medford, Oregon
Providence is calling a Licensed Practical Nurse to its Hospice team at Providence Medford Medical Center in Medford, OR. This is a full-time, day shift position. Our Caregivers travel throughout the Medford (Jackson and Josephine County) service area to care for Hospice patients. Travel will include visits to patient homes, facilities, and the medical center. Mileage reimbursement is granted.
In this position, you will:
Work under the direction of a Registered Nurse to provide quality nursing care to patients in their homes.
Assist with collection of assessment data and provide input to the planning of care.
Implement plan of care within the scope of licensure.
Required qualifications for this position include:
Graduate of an accredited school of practical nursing.
Licensure by the Oregon State Board of Nursing
Current American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider -or- Advanced Cardiac Life Support (ACLS) within 30 days of hire.
Valid drivers license and reliable transportation.
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire.
In compliance with the Providence Health & Services-OR, criminal background check is required for this position, based on assigned duties.
