We are seeking a Clinical Pharmacy Specialist - Mmc Primary Care that will be responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients referred to the Providence Health System's Anticoagulation Clinic. In addition, the CPS will serve as a resource to the pharmacy, nursing and medical staffs within the field of anticoagulation.
In this position you will have the following responsibilities:
Provide collaborative drug therapy management to patients referred to the anticoagulation clinic to include:
Initiation of warfarin
Warfarin dosage adjustment
Protime monitoring using the Coaguchek Plus protime monitor
Patient and family education
Drug/drug interaction monitoring
Drug/food interaction monitoring
Coordination with home health services as applicable in order to assure optimum patient outcomes
Effectively communicate and document all activities occurring in the anticoagulation clinic records to include:
Anticoagulation Plan of Care
Anticoagulation Progress Notes
Communication Notes/Special Instructions
Laboratory Quality Assurance Records
Anticoagulation Teaching Records
Participate in all required training and proficiency testing for the laboratories point of care testing program
Coordinate ongoing quality assessment and improvement activities for the anticoagulation clinic
Provide quarterly assurance reports to the P&T Committee
Serve as a resource to the pharmacy, nursing and medical staffs in the area of anticoagulation
Required qualifications for this position includes:
Graduate of an accredited college of pharmacy
Current valid pharmacy license by the Oregon Board of Pharmacy
Preferred qualifications for this position includes:
Master's Degree with residency -OR- equivalent education/experience
Experience in the area of anticoagulation dosing and monitoring
Two (2) years of experience in hospital or ambulatory care pharmacy
