Providence Health & Services Clinical Pharmacist - Inpatient in Medford, Oregon

Description:

Providence is calling a Clinical Pharmacist - Inpatient (1.0 FTE, Rotating/Variable ) to Providence Medford Medical Center in Medford, OR. This position will be mostly M-F with a weekend rotation (frequency varies based on need), 5 8-hour shifts. The shift is mostly days with about 1/3 of the time being evening shifts. There is also potential for night shifts if there is need.

We are seeking a Clinical Pharmacist - Inpatient that will be responsible and accountable for providing clinical, distributive and consultative pharmaceutical care to the community, home care and hospital services

In this position you will have the following responsibilities:

  • Provide quality patient care in an efficient, caring manner

  • Adapt to fluctuating workload

  • Carry fair share of workload

  • Utilized non-patient care time to maintain department. Needs little or no direction in accomplishing daily work

  • Drug orders are processed accurately in a timely manner

  • Utilize information systems timely and accurately

  • Properly document clinical interventions and drug information requests

  • Review patient records and orders regarding drug therapy taking into consideration age and disease state

  • Recommend and initiate changes as appropriate; is successful in improving patient outcome through monitoring, intervention and patient education consistently

  • Initiate and effectively utilizes problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality

  • Attend 80% of staff meetings. Maintains current knowledge of memos distributed and relevance to daily operations

  • Maintain education/in-services file current

  • Meet department and hospital education/in-service requirements

  • Adhere to hospital policies and procedures as well as state and federal laws

  • Review and interpret physicians orders to assure that the drug, dosage form, and dose is appropriate for the patient’s age, weight, and comorbidities

  • Check for drug incompatibilities and allergies, and take appropriate action when necessary

  • Review patient records and orders regarding drug therapy, recommends and initiates changes that are consistent with the patient’s age; is successful in improving patient outcome through monitoring and intervention consistently

  • Regularly consult with physicians and nursing staff members concerning the use, availability, dosage form, side effects and possible incompatibilities of new as well as currently used drugs; always take time to clearly explain information to concerned patients

  • Follow proper computer order entry procedures

  • Mix IV additive solutions and chemotherapy as required using aseptic technique within the laminar flow hood

  • Be responsible for the completion of all assigned work procedures during the shift, while maintaining the highest standards of quality

  • For medications defined as part of PHS’s clinical services (DUE, pharmacokinetics)

  • Concurrent interventions and follow-up will be made in order to assure the appropriateness, safety, and efficacy of the medication

  • Supervise and review work performed by supportive personnel

  • Use pharmacokinetic principles in establishing patient specific dosage regimens as appropriate and effectively documents all clinical monitoring

  • Understand the principles of the hospital formulary and follows formulary procedure

  • Maintain current awareness of and complies with the Quality Management programs, including awareness of current monitors, and participates as indicated

  • Document all clinical interventions and workload statistics

Qualifications:

Required qualifications for this position includes:

  • Licensed by the Oregon State Board of Pharmacy

Preferred qualifications for this position include:

  • One (1) year hospital experience

  • Completion of PGY-1
