Providence Clinical Liaison RN in Medford, Oregon
Providence is calling a Liaison Clinical - Registered Nurse (0.8 FTE, Days) to Providence Medford Medical Center in Medford, OR.
We are seeking a Liaison Clinical - Registered Nurse who will represent Providence home health and hospice (Home Services - HS) at patient care facilities and is a major access point for all customers; to include, but not limited to: hospital discharge planners/case managers, potential patients and their family members, physicians, physician office staff, service inquiries, etc.). The role of the RN Clinical Liaison is to coordinate and arrange home care services between HS department and patient care facilities for home health and hospice associated with a clinical referral. This position requires strong communication and interpersonal skills.
The position acts as a clinical resource and educates hospital staff regarding the services offered by HS, and contributes to the patient care plan as it relates to the discharge plan. The RN Clinical Liaison provides information to ensure a smooth transition for patients and their families following hospitalization. With new referrals, this position will coordinate prescribed services with the physician, payor, patient/caregiver, provider and department to ensure that services are provided in a timely and coordinated fashion.
In this position you will have the following responsibilities:
Coordinate care for all services being provided after the case manager/discharge planner provides the patient with freedom of choice for a home services agency and contacts the HS Liaison with an order for Home Services. Anticipate supply and equipment needs required to provide home services.
Facilitate streamlining of internal referral process so that one call initiates timely and appropriate admissions. Coordinate with HS scheduling and insurance verification team to determine start of care date for home services.
Meet with the patient to explain home services to be provided and obtains necessary patient information to arrange and provide care. Assess that services ordered are appropriate. Depending upon service ordered, information and education is provided about home health or hospice services, home health or hospice medicare benefit. Communicates information to patients/caregivers, physicians, and providers and assists in making alternative arrangements as dictated by insurance coverage and benefits.
Upon MD referral, provide hospice information, hospice evaluation, or, complete initial hospice admission process.
Document in the hospital record the plan of care for discharge with the name of the agency, phone number, anticipated start of care date and services ordered.
Enter patient data and appropriate documentation into the appropriate Home Services databases.
Keep current regarding community health issues and home health and hospice regulation changes.
Communicate with others in a warm, helpful, and professional manner while simultaneously building credibility and rapport. Establish and maintain positive working relationships with home care providers, payors/case managers, physicians and their office staff, hospital and extended care facility care managers and discharge planners, and Providence Home Services departments. Give high priority to customer satisfaction and anticipates customer needs.
Required qualifications for this position include:
Current Oregon RN license
Current Basic Life Support (BLS) at time of hire
Valid drivers license and driving record consistent with Safe Driving Policy 330 Incident Point Matrix at time of hire
1 year Full-time experience in home health or hospice
Experience in home care and in positions that required extensive customer service skills
Preferred qualifications for this position include:
Bachelor's Degree in Nursing
1 year Full-time experience in home infusion
1 year Full-time experience in discharge planning in an inpatient setting
