Providence Health & Services Clinical Case Manager in Medford, Oregon
Providence is calling a Clinical Case Manager (1.0 FTE, Days) to Providence Plaza South in Medford, OR.
In this position, you will:
Provide case management and care coordination services for at-risk patients, including patients with complex medical and psychosocial issues.
Use population-based disease management approaches and application of clinical protocols which are integral to the program.
Serve as a core member of the patient centered medical home teams led by the physician/provider.
Work towards the departmental goals to assure a systematic approach to the coordination of health services for patients along the care continuum in order to improve the patient experience, contain health expenditures and improve quality.
Assist providers in effectively identifying needs and coordinating access to appropriate care and services in an effort to improve or maintain the patient's functional status and independence.
Required qualifications for this position include:
Current state of Oregon Registered Nurse (RN) licensure and graduation from an accredited school of nursing, -or-
Master's of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work education, plus a Licensed Clinical Social Worker (LCSW) licensure
Washington licensure for Case Managers assigned to work in the state of Washington
3-5 years of clinical experience
Working knowledge Medicare criteria for skilled nursing services or home health/medical equipment services
Demonstrated ability to work cooperatively in a multi-disciplinary team
Preferred qualifications for this position include:
For RNs, graduation from a Bachelor's of Science in Nursing (BSN) program
Current recognition by a national professional organization in case management
Home health, skilled nursing facility and/or community health nursing (OR) clinical social work
1 year of managed care experience and/or utilization management experience
